This is the villian that appeared in the episode "Arthurs Tooth". He was the enemy in the flighty fluoride scour rangers. They defeated the plaque monster by wraping him in flossing thread and tossed him down a pit. As you can see in the stoumach part, there are eyes and mouth holes. This proves it is a dressed up human.
